Surgical Advances for Hip Fractures in the Elderly

Abstract

Objective: Hip fractures in the elderly are associated with factors such as osteoporosis and falls. Their treatment involves a surgical approach, with clinical stabilization being essential to reduce complications. This study analyzes new internal fixation techniques and their advantages over arthroplasty. Methods: It is a systematic review following PRISMA, with searches in databases such as MEDLINE and BVS. Studies from 2019-2024 on elderly patients (≥60 years) with hip fractures were included, analyzing intra- and postoperative complications. Results: About 70% were retrospective studies analyzing surgical treatments, 10% were systematic reviews or meta-analyses, and 10% were clinical trials. The choice of treatment for hip fractures depends on the patient's profile. Total hip arthroplasty (THA) showed better functional outcomes in young and active patients, while hemiarthroplasty (HA) was more suitable for frail elderly individuals. Internal fixation (IF) was advantageous for non-displaced fractures but had a higher reoperation rate. Conclusions: Approaches such as cemented HA benefited osteoporotic patients, while innovative techniques like Talon-PFN and SuperPath showed potential. Early surgery improves outcomes, reduces complications, and should be personalized according to age, comorbidities, and mobility.
