Introduction: Although treatment for maternal syphilis is available, this infection is still vertically transmitted to thousands of children. Objective: To examine the obstacles in the diagnosis and adequate treatment of maternal syphilis and the consequences of vertical transmission of this infection on the quality of life of affected infants. Methodology: Scoping review of the PubMed database, including publications from 2020 to 2024. Opinion articles and case reports were excluded. Results: The occurrence of congenital syphilis (CS) is related to difficulties in accessing quality health and prenatal care services, hindering proper maternal diagnosis and treatment. This condition significantly impacts the quality of life of affected children, as they may present anemia, splenomega ly, hepatomegaly, dental and bone abnormalities, deafness, intellectual disability, and neurosyphilis. Conclusion: CS and its consequences can be prevented through early diagnosis and treatment of maternal syphilis. Therefore, public policies that ensure better access to quality prenatal care are essential for improved diagnosis, treatment, and prevention of CS.
