Accuracy of Transcranial Doppler in the Assessment of Intracranial Hypertension in Acute Brain Trauma

Abstract

Introduction: ICH is a severe TBI complication linked to cerebral edema and high mortality. TCD offers a non-invasive alternative to estimate ICP and CPP. Objective: Assess TCD accuracy in monitoring ICH in acute TBI. Methodology: Integrative review of 17 articles (2020–2025) from PubMed, BVS, and CAPES. Results: TCD indices correlated with elevated ICP, especially PaR (AUC 0.77). Combined with ONSD, accuracy reached up to 0.943. Discussion: TCD is promising when paired with other tools. Limitations include operator dependency and lack of standardization. Conclusion: TCD shows potential but should be used cautiously until further validation.
