Hipotermia terapêutica no pós-parada cardiorrespiratória

Objetivo: Esta revisão narrativa investigou o impacto da hipotermia terapêutica (TTM) na recuperação neurológica de pacientes após parada cardiorrespiratória (PCR). A pesquisa foi realizada no PubMed (2019–2025), identificando 27 artigos; após seleção, nove estudos publicados entre 2019 e 2023 foram incluídos, conduzidos na Coreia do Sul, Estados Unidos, Canadá, países europeus e Polônia. Os trabalhos apresentaram diferentes delineamentos e abordaram comparações entre faixas de temperatura alvo (32–36,5 °C), tempo de indução, duração da manutenção e tipos de dispositivos de resfriamento. Os achados indicaram benefício consistente do controle da temperatura, especialmente em valores próximos a 33 °C. Fatores como maior tempo dentro da faixa-alvo, indução precisa e manutenção por pelo menos 24 horas estiveram associados a melhores desfechos neurológicos. Foram relatadas complicações incluindo convulsões, infecções e alterações metabólicas, o que ressalta a importância de monitoramento rigoroso. As principais limitações envolveram amostras reduzidas, predominância de estudos observacionais, protocolos heterogêneos, ausência de randomização e carência de acompanhamento. Em conclusão, a TTM configura-se como uma medida neuroprotetora importante no pós-PCR, e deve compor os protocolos assistenciais quando aplicada de forma adequada; mas, sua consolidação como prática padronizada depende de ensaios multicêntricos prospectivos com desfechos clínicos e funcionais mais robustos.
